A Comparative Study On Management Of Colles’ Fracture By Closed Manipulation And Casting Versus Closed Manipulation, K-Wiring And Casting

Abstract
Background: Colles’ fracture is one of the common injuriesin Orthopaedic practice. Since most of these are commonly managed by closed manipulation and casts, these invariably unite in malpositiondue to loss of reduction within plaster and therby leading to malunion and stiffness. To overcome that problem percutaneous K-wire fixation following reduction is being advocated in Colles’ fracture. Aim: To compare the outcome between the simple manipulation and casts with manipulation, percutaneous K-wire fixation followed by cast in Colles’ fracture. Settings And Design: Comparative study done in a tertiary care institute from January 2014 to December 2015. Materials and Method: 42 patients who fulfilled the selection criteria were allotted randomly to two groups. The group A received closed manipulation and below elbow cast under anesthesia with C-Arm guidance. Patients in group B received closed manipulation, percutaneous K-wire fixation under C-Arm guidance and below elbow casts.These patients were followed up at regular intervals till 1 year with roentgenograms and functional evaluation. Functional outcome was evaluated using Gartland and Werley demerit scoring system. The scores in both the groups were compared and analysed. Results: Patients in group B had a better grip strength, range of motion of wrist, radiological parameters and Gartland and Werley scores as compared to the group A. Conclusion: K-wire fixation of Colles’ fracture after closed manipulation and immobilization with cast showed a better outcome in both radiological and functional grounds, since it maintains the reduction within the plaster.
