A Study of Metabolic Syndromes and Insulin Resistance in General Population

Abstract

BACKGROUND: It has been witness that there is a paradigm shift of medical science in its therapeutic and preventive approaches for non-communicable diseases (NCDs). The research regarding NCDs focuses on detection of genetics, cellular, metabolic and biochemical mechanisms and processes so as t arrive at the ability to forestall their progression to clinical illness. MATERIALS: 300 patients above 18 years of age not previously known to have diabetes or hypertension or any significant disease. Measuring prevalence of metabolic syndrome(MS) – by help of ATP III revised criteria 2005 Calculation of Insulin Resistance(IR) - The four different scores used were 1) Insulin Sensitivity Index (ISI) 2) American Diabetes Association Score (ADA) 3) Finnish Diabetes Risk Score 4) Indian Diabetes Risk Score (IDRS) OBSERVATIONS: In our study Maximum patients are in 20-39 years of age In study male patients were 63% and female were 37%. Prevalence of IR (40%)by ISI prevalence of MS (26%) revised ATP III clinical criteria. 1) MS has 51% prevalence in age group of 40-59 years. 2) High prevalence of MS in female population (37%) than in male population (19%). 3) Mean age of patients of High IR is 45 years. 4) Mean Systolic BP of patients with MS is 149 mmHg and mean Diastolic BP in normal individuals is 92 mmHg. 5) Mean of Systolic & Diastolic BP of patients with Low IR are 125 & 80 mmHg respectively and mean of Systolic & Diastolic BP in High IR are 139 & 88 mmHg respectively. 6) Mean Waist circumference of patients with MS is 87 and in normal patients, mean Waist circumference is 80. 7) Mean Waist circumference of patients with Low IR is 78 and in High IR mean Waist circumference is 89. 8) Mean Fasting blood sugar of patients with MS is 130 and in normal patients, mean Fasting blood sugar is 85. 9)Mean Fasting blood sugar of patients with Low IR is 86 and in High IR mean Fasting blood sugar is 103. 10) Mean Triglycerides of patients with MS is 174 and in normal patients, mean Triglycerides is 124. 11) Mean Triglycerides of patients with Low IR is 125 and in High IR mean Triglycerides is 156. 12) Mean HDL cholesterol of patients with MS is 34 and in normal patients, mean HDL cholesterol is 57. 13) Mean HDL cholesterol of patients with Low IR is 55 and in High IR mean HDL cholesterol is 48. CONCLUSION: Significantly high prevalence of metabolic syndrome and insulin resistance in apparently normal individuals.
