Age-related and hormonal and metabolic peculiarities of patients with non-alcoholic fatty liver disease in combination with subclinical hypothyroidism

Abstract
Objective — to identify age-related and hormonal and metabolic features in patients with non-alcoholic fatty liver disease (NAFLD) in combination with subclinical hypothyroidism (SH). Materials and methods. The study involved 185 patients with NAFLD, who were divided into two groups: group 1 — patients with NAFLD (n = 57), group 2 — patients with NAFLD combined with SH (n = 128). All patients underwent the complex clinical and biochemical investigations, enzyme immunoassay and instrumental study. Results. It was revealed that in the early stages of SH formation in patients with NAFLD with a level of TSH > 4 µMU/ml, significant changes were observed in the indices of the lipid, carbohydrate profile and liver functional state compared with patients who have TSH < 4 µMU/ml. The depth of hormonal and metabolic changes affects the course of NAFLD in combination with SH and leads to the formation of intracellular cholestasis. In patients with NAFLD in combination with SH under 50 years old, the metabolic changes are formed that depend on the TSH levels, and are characterized by a reliable isolated increase in the GGT level, while in patients older than 50 years, the disorders of carbohydrate profile prevail against the background of progressive changes in the liver tissue. Conclusions. taking into consideration that thyroid dysfunction affects the rate of NAFLD formation and progression in patients under 50 years old, it is extremely important to conduct to start early prophylaxis measures to prevent the vascular aging and the development of cardiovascular diseases and their complications measures to.
