An Epidemiological Study Of Carcinoma Of Cervix Using Papanicolaou Smear As A Screening Procedure

Abstract
This paper attempts to review the available knowledge regarding the epidemiology of cervical cancer and evaluating the importance of Pap smear as a screening procedure. MATERIAL & METHOD: This was a prospective study of 500 women above 25 years which was performed in the District Hospital Chhindwara for a period of three years from 2015-2017. First of all Gynecologist recorded the detailed clinical History of all the women attending the Gynaecologic OPD regarding the age, parity, socioeconomic status, ethnicity and the presenting complaint. After that Pap smear was collected with the help of Ayre’s spatula and sent to the pathology laboratory, department of Pathology District hospital Chhindwara, where it was stained by Pap stains and reported according to Bethesda System. RESULTS: Out of 500 Patients Incidence of Low grade Squamous Intraepithelial lesion (LSIL) was 3.4 %, High grade Squamous Intraepithelial Lesion (HSIL) was 1.6 % and Squamous Cell Carcinoma (SCC) was 0.4 %. It was observed that commonest age for dysplasia was 35-40 years while that of malignancy was 45 - >50 years. Maximum number of cases belonged to low socioeconomic group. Multiparity plays an important role in the causation of cervical cancer as most of the cases fall in this category. Commonest presenting complaint among the abnormal smear was bleeding Per Vaginum specially the postcoital bleeding and the postmenopausal bleeding. CONCLUSION: In any case since prevention is always better than cure. It is better to take preventive measures in early stages of cancer rather than facing the consequences. Preventing and treating cervical cancer and reducing the burden are possible by targeting resources to the areas with high prevalence. Pap smear appears to be the most practical method of early detection of Carcinoma of Cervix hence it should be established as a routine diagnostic aid in the Gynaecological OPD.
