AN INTERESTING CASE OF SEVERE HEMOPHILIA COMING FOR EMERGENCY CLOSED REDUCTION AND TENS NAILING OF FRACTURE FEMUR.

Abstract

Haemophilia A is a X linked recessive disorder with occurrence of about 1 in 10,000male birth.factor VIII levels <1% was considered as sevre diseae with risk of spontaneous bleeding.A 9 year severe haemophiliac boy came with fracture femur for emergency closed reduction and TENS nailing. 3 injections of 250 units factor VIII required to increase to100%. (Required Units = body mass (kg) x 0.5 x desired factor VIII increase (% of normal).Factor VIII level came up to 26%.patient required further 1400 units to be transfused 30 min prior to surgery. Surgery was done under general anaesthesia with universal precautions and close monitoring of intraoperative bleed. Transfusion of factor VIII continued in the postoperative period at the rate of 50units/kg after 12hrs and 25units/kg 12hrly for 2-7days followed by 10-15units /kg 12hrly for next 3-7days.This emphasis on the importance of continous monitoring of the factor levels.
