Analysis of bile culture for bacteriological profile and its antibiotics sensitivity pattern in patients undergoing ERCP.

Abstract
Introduction: In normal healthy person bile is sterile in biliary duct. Bacterial Infection of obstructed bile leads to cholangitis [1]. Choledocholithiasis, malignant stricture and benign biliary stricture are common predisposing factors for cholangitis [2,3]. This category of patients needs empirical treatment with broad spectrum antibiotics and decompression of biliary system [2]. Although blood culture help in knowing bacteriological agent but it is only positive in 50% of cases [3] while bile culture is positive in 95% of patients [4]. Widespread and injudicious use of antibiotics, needs evaluation of antibiotics sensitivity pattern of causative micro-organism [5,6]. Methods: Study was conducted in department of Gastroenterology, Indira Gandhi Institute of Medical Sciences, Patna. It is a retrospective study and data was collected from Oct. 2013 to Dec. 2014. Bile samples were obtained from all patients undergoing ERCP for various diseases and sent for bacteria culture and sensitivity. Bile was collected using cannula. Five to 10 ml bile collected in sterile syringe and send immediately to microbiology department. Provisional diagnosis of cholangitis was made based on patient symptoms, signs and radiological evidence of biliary tract obstruction which was later confirmed by blood/bile culture. All patients of cholangitis received antibiotic prior to cholangiography procedure. Results : A total 117 patients screened for data collection out of these 36 excluded due to unavailability of bile culture sensitivity report. A total 81 patients data was analysed. Out of 81, 58 were female and 23 were male (71.6% vs 26.4%) with median age 38.6yrs (18yrs-67yrs). Benign biliary diseases was noted in 46(56.8%) while malignant disease was present in 31(43.2%) of patients. Out of 46 benign biliary disease patients 38 have choledocholithiasis. In malignant biliary disease, carcinoma gall bladder was most common indication of ERCP. Choledocholithiasis (46.91%) and carcinoma gall bladder (22.22%) were the most common indication for cholangiographic study
