Analysis of risk factors in children treated for acute lymphoblastic leukemia in bydgosko-kujawski region in 1976-2010

Abstract

Introduction. Acute lymphoblastic leukemia (ALL) is the most frequent pediatric malignancy. ALL has been treated with multi-agent chemotherapy from early sixtes.Aim. Analysis of prognostic factor in treatment of childhood ALL in bydgosko-kujawski region in 1976-2010.Material and methods. A total number of 395 children with ALL were included into the study. With respect to time period and therapy protocol, the patients were divided into 5 groups. Risk factor analysis was performed in each group.Results. Prognostic factor in childhood ALL have been changing over time: some of them have lost their significance, while new factors have appeared. The following factors had adverse prognostic impact on survival: central nervous system involvement in groups 1 and 2; poor response to steroids at 8 day of therapy in group 3; T-cell immunophenotype and splenomegaly in group 4a; lymph node involvement and M2/M3 marrow at 15 day of therapy in group 4b; and BCR-ABL rearrangement in group 5, while age 1-6 years at diagnosis was the only positive prognostic factor in group 2.Conclusions. In recent protocols immunophenotype, initial white blood cell count, BCR-ABL rearrangement, and response to therapy are independent prognostic factor. These factors are used for stratification to treatment groups, and stem cell transplantation.
