Analysis Of Tuberculosis Related Mortality At GHTM, Tambaram Sanatorium

Abstract
Aim To analyse the demographics and causative factors of Tuberculosis related mortality at Government Hospital of Thoracic Medicine (GHTM) , Tambaram Sanatorium. Methods and Material Retrospective analysis of mortality data over a period of six months were studied using the case records and death certificates.Results During this period there were 350 deaths at GHTM, Tambaram sanatorium of which 202 patients died due to Tuberculosis. Most TB deaths were in the males (87%) of productive age group 40 to 60 years(51.9%) and 20 to 40 years(36.1%). 91% of the mortality were due to Pulmonary and Disseminated forms of TB while 9 % were due to extrapulmonary Tuberculosis predominantly TB meningitis(7.9 % of overall mortality). Sputum positivity was 75 % among the mortality. Most common comorbidities observed included DM(28%) and HIV(25%). 59 % of mortality occurred within the first one week of hospitalisation reflecting the advanced nature of disease and late referral in such cases. Among the mortality 48% of deaths were among patients never exposed to ATT, 43.5 % had previous exposure to ATT, MDR TB was 6.9 % and XDR 1.5 %. The commonest cause of death identified was Respiratory failure (56.5 %) reflecting the destruction of pulmonary parenchyma and poor respiratory reserve. Conclusions Analysis of mortalilty data reiterates that lives are still lost due to Tuberculosis and reflects the socioeconomic loss due to deaths in the productive age group. Need for early identification of Tuberculosis, HIV-TB coinfection, control of Diabetes mellitus and early referral is the need of the hour.
