Application Video Head Impulse Test in the diagnosis of balance system in children

Abstract

Introduction. One of the new methods of testing the balance system is VHIT (Video Head Impulse Test). This test is used in the diagnosis of peripheral damage to the balance system using the vestibulo-ocular reflex. Vestibulo-ocular reflex is eye movement in the opposite direction to the direction of rotation of the head. It is observed it in newborns, and normalized to 2 months of age and matures within the first two years of life. VHIT test allows to determine the site of injury within the channel vestibular system.Material and method. The study included 28 patients aged from 5 to 18 years, including 9 children with vertigo and 19 healthy children. In all patients underwent VHIT with registration vestibuloocular reflex and evaluation ocular lag factor (gain). Beyond that patients with dizziness we performed full videonystagmography test.Results. In the group of healthy children gain the correct results were observed in 14 patients. In the group of children with vertigo on the basis of VNG test we diagnosed damage of the peripheral part of the balance system in 2 children. VNG test results in these patients, coincide with the results of the VHIT test for individual lateral semicircular canals. We observed lesion of central parts of the balance in 7 children. In 4 patients in this group were observed incorrect VHIT test results.Conclusions. VHIT test allows perform rapid and non-invasive screening analysis of vestibulo-ocular reflex, and detect damage of the individual semicircular canals.VHIT test can complement the diagnosis of vertigo in the clinical setting, as well as be an option for children who do not tolerate standard VNG testing.
