Articulators as diagnostic tools in orthodontics – review of literature

Abstract
Introduction. An articulator is defined as a mechanical instrument that represents the temporomandibular joint and jaws, to which maxillary and mandibular casts may be attached to simulate mandibular movements. The conducted studies available in the literature show that about 21% of orthodontists mount models routinely, 44% occasionally, and 35% never. The use of articulated study models has been demonstrated by a number of clinicians to aid diagnosis and treatment planning of orthodontic cases. Aim of the study. To review current literature on the use of articulators in the diagnosis and treatment planning in patients undertaking orthodontic treatment. Materials and methods. A review of the literature using MEDLINE database concerning the indications of articulators in orthodontics prior to orthodontic treatment in the following clinical situations: 1. Large discrepancy (more than 2 mm) between the retruded contact position and the intercuspal position. 2. Orthodontic cases with multiple missing teeth in which a stable intercuspal relationship cannot be recorded. 3. Cases undergoing maxillary and bimaxillary orthognathic procedures. 4. Temporomandibular joint dysfunction before orthodontic treatment. 5. Evaluation of teeth contact including occlusal interferences. 6. Diagnosis and treatment planning of incorrect functional occlusion. Results. Consistent and differing views are presented in the assessment of the use of articulators for the diagnosis and treatment of orthodontic patients. Conclusion. The use of articulators in orthodontic practice is a source of new parameters on the maxillomandibular complex, allowing proper evaluation of the orthodontic treatment needs.
