Bezpośrednie wprowadzenie trokarów z uniesieniem pochewki mięśnia prostego brzucha: nowa technika w chirurgii bariatrycznej

Abstract
Wprowadzenie: Pierwotne wprowadzenie trokaru stanowi pierwszy etap każdego zabiegu laparoskopowego i wiąże się z wieloma powikłaniami. U pacjentów z otyłością olbrzymią wprowadzenie trokara wiąże się z większą liczbą powikłań w porównaniu do pacjentów bez otyłości. Materiał i metody: W naszym badaniu opisaliśmy zastosowaną przez nas technikę pierwotnego wprowadzenia trokaru poprzez bezpośrednie wprowadzenie go z uniesieniem pochewki mięśnia prostego brzucha przy pomocy pojedynczych kleszczyków do serwet typu Backhaus, a następnie oceniliśmy bezpieczeństwo i skuteczność jej stosowania w chirurgii bariatrycznej. Dyskusja: Wyniki wskazują, że uzyskanie pierwotnego dostępu z zastosowaniem naszej techniki pozwala na skuteczne przeprowadzenie laparoskopowych zabiegów bariatrycznych. Przyjęta przez nas technika stanowi bezpieczny i skuteczny pierwszy etap zabiegów laparoskopowych u wszystkich pacjentów, przy czym jedynym przeciwwskazaniem jest ciężka hepatomegalia.<br/><br/>
