Carotid artery stenting – 15 years of single centre experience

Abstract
<b>Introduction. </b>About 20% of ischemic strokes are caused by extracranial carotid artery stenosis, mainly due to atherosclerosis. Patients with hemodynamically important carotid stenoses need invasive treatment to prevent disability and possible death. Many trials done in the last years have proved high efficacy of both, surgical endarterectomy and minimally invasive endovascular stenting in ischemic stroke prevention. The aim of our study is to present the data of carotid stenting registry from Lublin centre done in the 15-year period.<br><b>Material and methods.</b> Between 1996 and 2011, a 1200 carotid stenting procedures were performed in 1115 patients. We stented only patients with 70% stenoses of internal carotid arteries. In our group most of patients were symptomatic – 1026 (92%). Technically, 96.1% stenting procedures were done with the use of neuroprotection – in 95.6% of cases the same type of neuroprotection was used (anti-embolic filters). The type of the stent used was also identical in most of our procedures – 88.7%.<br><b>Results.</b> Technical success of all carotid stenting procedures was 99.8%. During a 30-day peri-procedural period, neurological complications occurred in 19 (1.7%) patients. There were 4 (0.3%) deaths, 3 (0.25%) completed strokes, 11 (1%) cases of TIA, and one hiperperfusion syndrome – totally, the periprocedural stroke and death rate was 0.63%. In the whole group of patients controlled after 12 months (94.6% of all stented) there were no deaths, 5 patients (0.5% of all patients) had completed stroke, ipsilaterally to the stenting procedure. TIA symptoms happened, or were suspected in anamnesis in 31 patients (0.3% of all patients). Evaluating stent patency – after 12 months, out of 987 examined patients, we found 7 cases of in-stent restenosis (0.6% of all treated patients). Among them, 5 patients were symptomatic again (TIA), because of hemodynamically important (about 70%) restenoses – in all these cases we repeated balloon angioplasty with good result.<br><b>Conclusions. </b>Endovascular stenting of internal carotid arteries stenoses of 70% or more in symptomatic patients is effective and safe.
