Clinical and radiological evaluation of the influence of general and local factors on the healing of post-apectomy bone tissue
Journal Title: Journal of Stomatology (Czasopismo Stomatologiczne) - Year 2012, Vol 65, Issue 3
Abstract
Introduction. The healing of post-resection intraosseous defects is modulated by the influence of numerous general and local factors. These factors may have an impact on the final effect of bone defect healing and also condition the healing process via the regeneration or reparation of lost tissue, i.e. the reconstruction of tissue continuity in the area of the defect, without restoring their diversification and functions. The aim of the study. The aim of the study was clinical and radiological evaluation of the influence of general and local factors on the healing of post-apectomy bone tissue. Material and methods. The studies were conducted on a group of 120 patients who had undergone endodontic and surgical treatment for chronic periapical lesions (120 bone defects). The study comprised 4 research groups: BG – (30 defects) – treated with resorbable collagen membranes, BOC (30 defects) – treated with xenogenic bovine material plus an addition of 10% porcine collagen, BOC/PRP – (30 defects) treated with a xenogenic bovine material with the addition of 10% porcine collagen with platelet rich plasma, C – (30 defects) treated without the use of regeneration techniques (control group). The influence of selected general and local factors on the healing of post-resection intraosseous defects was assessed 6 and 12 months after the procedures using logistic regression and intra-group analysis. Results. Logistic regression analysis revealed that the presence of transosseous defects as well as the treatment method used in the procedure had an influence on the bone healing process during both control periods. In addition, periosteal location had an influence on the final defect healing outcome 6 months after the procedure, and soft tissue healing type and the sex of the patient were key factors after 12 months. Intra-group analysis revealed that the presence of transosseous defects and the location and state of the periosteum were influential factors. Conclusions. The presence of transosseous defects had the biggest influence on the final treatment effect.
