Comparison between the Clinical Outcome of Anterolateral and Posterior Approaches to the Hip Joint in Hip Joint Arthroplasty

Abstract
Objective: The objective of this study was to compare the clinical outcome of anterolateral versus posterior approach to the hip joint in hip joint arthroplasty. Methods: The prospective study was carried out on 52 patients who suffered from traumatic and degenerative conditions of the hip joint. e patients were treated by hip joint arthroplasty (bipolar hemiarthroplasty/total hip arthroplasty) through anterolateral approach or posterior approach. 34 patients were operated using the posterior approach and 18 patients were operated using anterolateral approach. e age of patients was ranged from 42 to 68 years. e patients were followed from the date of discharge to the end of 6 months from the date of surgery. Results: The average length of hospital stay was 11.74 ± 1.78 days for the posterior group compared to 10.94 ± 1.47 days for the anterolateral group. e means of the scores of both the groups at the end of 6-month follow-up period using the Modied d’Aubigne and Postel method were compared and were found to be signicantly different. e anterolateral group had a mean score of 36.22 ± 1.17 compared to the mean score of posterior group which was 35.35 ± 1.28. 2 patients of 34 (5.88%) from the posterior group experienced post-operative dislocation. No patient in the anterolateral group experienced post-operative dislocation. Conclusions: In the hands of an experienced surgeon, both approaches were found to be excellent for hip joint arthroplasty as the mean functional outcome score was 36.22 ± 1.17 for the anterolateral group and 35.35 ± 1.28 for the posterior group, both were more than 32, that is, excellent outcome. Anterolateral group showed beer post-operative recovery as compared to the posterior group. Dislocation was found to be a complication in the posterior group with a dislocation rate of 5.88%.
