Comparison of clinical examination, USG, USG guided FNAC study of cervical lymphnodes in oral malignancy

Abstract
Background: Various tools for the staging of lymph nodes used are clinical examination, ultrasonography, USG-guided fine needle aspiration cytology. In rural setup, USG and USG FNAC are commonly used along with clinical examination as these are easily available, inexpensive, minimally invasive. Aim: To compare clinical examination, USG and USG-guided fine needle aspiration study in oral squamous cell carcinoma. Material and Methods: A total of 50 patients of diagnosed squamous cell carcinoma of oral cavity were evaluated by clinical examination, USG and USG FNAC. Then patients were subjected to the proper treatment after thorough evaluation. After surgery neck nodes were dissected and sent for histopathology. From this sensitivity, specificity, accuracy, positive and negative predictive values were derived for each modality and compared with each other and with histopathology. Results: The sensitivity of all three modalities was equal i.e. 77.77%, but specificity and accuracy of USG FNAC as compared to clinical examination and USG was definitely higher i.e., 84.37% and 82%. USG was having definitely higher specificity 78.12% and accuracy 78% than palpation, which has specificity 75% and accuracy of 76%. Conclusion: As specificity of these modalities when combined together was 90%, it is worthwhile to go ahead with similar approach for further few years so that the experience gathers and technology gets well established.
