ComparisonofTwoDosesofCisatracurium(0.15mg/Kgand0.2mg/Kg)inaSingleCentre,Prospective,Comparative,ParallelGroupofPatientsUndergoingGeneralAnesthesiawithEndotrachealIntubationbyStudyingEaseofIntubation

Abstract

Objective:Studytheadvantagesandpitfallsofboth0.15mg/kgand0.2mg/kgofCisatracuriumforIntubation.Background:ThepatientscomingtoaTertiaryCareHospitalduringperiod2018-2019forsurgeryunderGeneralAnaesthesia,whomettheInclusionCriteriawerestudiedinahundredpatients.Aftergivingallcomponentsofbalancedgeneralanaesthesia,patientsrandomisedinto2groupseither0.15mg/kgor0.2mg/kgofCisatracuriumasamusclerelaxantandIntubationwasdoneat3minutes.MeasurementsandResults:Heartrate,Bloodpressure,Systolic,Diastolic,Mean,SpO2,ETCO2,TOFmonitoringattheadductorpollicis,andBISweremonitoredfor7minutesandIntubatingconditionsinboththegroupswereevaluated,andscoredaccordingtotheSteyn’sModificationofHelbo-HansenScoringsystem.InhaemodynamicchangesinGroupA[0.15mg/kg]HeartrateandBloodpressureshowedanincreasingtrendbeforecomingdowntobaselineby7minutes.InGroupB[0.2mg/kg]thetrendwasdecreasingfrombaselineafteraminorincreaseinheartrateat2minutesandMeanBloodpressurewasloweratallintervalswhencomparedtobaseline.RegardingIntubationInGroupAmaximumnumberofparticipantswerehavinggoodscore(56%)followedbypoor(30%)andexcellent(14%)whereasinGroupBexcellentscorewasmaximum(62%)followedbygood(32%)andpoor(6%).WithrespecttoTrainoffourstimulationGroupBshoweddecreasedresponsetoneuromuscularstimulationfrom3minutesonwardscomparedtoGroupA.Bispectralindexmonitoring,inbothgroups,showedadecreasingtrendateverytimeinterval.Conclusion:Cisatracuriumisapotentnon-depolarisingneuromuscularblockingagentandtrachealintubationcanbeaccomplishedwithgoodtoexcellentintubatingconditionsaccordingtoSteyn’sModificationofHelbo-HansenScoringSystemat3minutesfollowing0.15mg/kgand0.2mg/kgofCisatracurium.Fromthestudyof100patientsin2groups,wefoundthatCisatracuriuminadoseof3ED95(0.15mg/kg)hashaemodynamicstabilityinviewofHeartrateandBloodPressure.Cisatracuriumatadoseof0.2mg/kgwasmorehaemodynamicallystablethanCisatracuriumat0.15mg/kgduringintubation.
