Complications of endoscopic therapy of pancreatic organized necrosis

Abstract

Introduction: Pancreatic fluid collection (PFC) may be a complication of acute bout of pancreatitis. Their common feature is the presence of encystic pancreatic juice with or without necrotic debris. Acute and chronic pseudocyst and organized pancreatic necrosis (OPN) are most frequently met in clinical practice. In last decades endoscopic drainage of OPN was introduced to the therapy. Aim of study: The analysis of complications of OPN endoscopic management. Material and methods: 46 patients with OPN were treated in Department of Gastroenterology and Hepatology of Medical University in Gdańsk. Endoscopic (transpapillary and transmural) and percutaneous drainage of PFC were performed. Results: ln 19 patients the following complications were observed: in 14 - infection of OPN, in 2 -perforation of gastrointestinal tract, bleeding from stomic hole in 5, pancreato-jejunal fistula in 2 and drain translocation in 3 cases. Because of these complications two patients were treated surgically. In one case the embolization of spienic artery pseudoaneurysm during angiography was performed. Conclusions: Endoscopic treatment of OPN is preferable if cooperation with surgeons and radiologists is accessible. It should be stressed that the risk of complications related to this method of management does not endanger patient's life.
