DEFENSIVE MEDICINE APPROACH IN PATIENT ASSESSMENT BY EMERGENCY DEPARTMENT PHYSICIANS

Abstract
OBJECTIVE: The most distinctive feature of defensive medicine is that the physician wants to protect herself against the risk of being sued by the patient and her relatives by performing medical procedures that are not necessary for diagnosis and treatment. Emergency services are one of the risky branches where defensive medicine is prevalent. In our study, we aimed to show in advance the legal, forensic-medical problems awaiting our country's emergency physicians by determining to what extent defensive medicine is practiced by emergency physicians, whether there are differences between academic titles, and what factors are related to them. MATERIAL AND METHODS: 387 physicians, including general practitioners, emergency medicine assistant physicians, emergency medicine specialists and emergency medicine faculty members working in the emergency departments of hospitals in our country, were included in the study. A survey form was created for each participant. Survey forms were delivered to participants online via the Google Drive system. RESULTS: It has been found that as pressures from the work environment increased., concerns about complaints and lawsuits and defensive medicine practices increased. In addition, it was determined in the study that as complaints and concerns about being sued increase, defensive medicine practices increase and the efficiency, effectiveness and quality of the healthcare system decreases. The study also showed that defensive practices increased as the pressure of media and social communication channels increased. In our study, it was also found that emergency department physicians applied positive defensive medicine more in all titles. CONCLUSIONS: Defensive medicine practices not only cause negativities for patients and physicians, but also lead to disruptions in healthcare delivery. For this reason, we think that the reasons for defensive medicine practices should be further examined in order to increase the efficiency of healthcare delivery and ensure the safety of patients and physicians.
