Dynamic Contrast Enhanced Breast MRI in Diagnosis of Breast Carcinoma: Comparison between the Accuracy of Kinetic Curves and Morphological Features

Abstract

Background: Breast carcinoma being the commonest and frequently a lethal female malignancy needs prompt radiological evaluation for proper characterization. MRI breast is highly sensitive for diagnosis and uses two features for assessment of any lesion; kinetic and morphological. Considering poor consensus between these two features, this study was conducted to determine diagnostic accuracy of each feature for detecting breast carcinoma. Patients and Methods: It was a cross-sectional study. One Hundred and fifty female patients with palpable lesions suspicious for malignancy or having suspicious mammographic findings or both, referred from surgical outpatients department (OPD) of Sir Ganga Ram Hospital were assessed using 1.5 Tesla MRI machine. The images were evaluated on morphological and kinetic basis and comparison of diagnostic accuracy was made. Results: The mean age of the patients was 52.5 years. Out of150 patients, 78.6% patients were found to have malignant lesions on Magnetic Resonance Mammography (MRM) while 77% patients proved to have malignant lesions on histopathology. Morphological analysis revealed 77% malignant lesions with sensitivity and specificity of 98.2% and 91.1%.Dynamic kinetic evaluation proved 76%malignant lesions with sensitivity of 93.9%and specificity of 85.2%. Conclusion: Morphological analysis of lesions on dynamic contrast-enhanced (DCE) Breast MRI has high sensitivity and specificity compared with kinetic features and can be used for prompt diagnosis.
