Enamel demineralization in patients treated with fixed orthodontic appliances – pre-evaluation of the effectiveness of resin infiltration treatment

Abstract
[b] Aim[/b]. White spot lesions (WSL) on smooth surfaces of the teeth are a frequent complication occurring in patients treated with fixed orthodontic appliances. The purpose of this study was to pre-evaluate the efficacy of treatment using the resin infiltration technique. [b]Material.[/b] Six patients treated with fixed orthodontic appliances, ages 12 – 24, diagnosed with WSL on smooth enamel surfaces of the teeth. The study comprised a total of 40 teeth. Method. The degree of demineralization was measured with a Diagnodent pen. The Hibst and Paulus scale was used to evaluate the results. Demineralized enamel was treated with resin infiltration technique (Icon, DMG). The aesthetic effect was evaluated by comparing the pre-treatment photographs with those taken after Icon application. A follow-up study was conducted after 6 months and the stability of the aesthetic and therapeutic effect was assessed. The patient’s satisfaction level was examined. [b]Results[/b]. 100 percent of patients were satisfied with the aesthetic result, even though on 23 teeth (57.5 percent of cases) their natural color was not restored over the entire tooth demineralization area. The demineralized areas have shrunk considerably; however, lines or dots of a whitish, lighter shade remained. These whitish discolorations were found in the areas where Diagnodent readings oscillated close to the border limits for invasive treatment. A follow-up confirmed the stability of the aesthetic effect and the absence of any WSL recurrence in the areas that underwent treatment. [b]Conclusions.[/b] Resin infiltration may be effective in the treatment of orthodontic complications in the form of white spot lesions. The effect is immediate and independent of the patient’s cooperation even though it may depend from the initial demineralization degree. It seems that the method may effectively bridge the gap between re-mineralization and micro invasive caries treatment, although it may depend on the initial level of demineralization.
