Estradiol level in patients with pulmonary tuberculosis

Abstract
Objective — to study the hormonal state of patients with pulmonary tuberculosis by the example of estradiol with its various forms and try to establish a possible relationship between the pathogenesis of tuberculosis and the level of the hormone. Materials and methods. Under the supervision of the authors, there were 64 patients with pulmonary tuberculosis, 18 of whom had sensitive TB and 46 — multidrug-resistant tuberculosis (MDR-TB). The control group consisted of 5 women and 8 men who have no illnesses of lungs and in the past there is no anamnesis. They were determined the level of estradiol to establish a normal index in our region. In women of the control group, the level of estradiol fluctuated within the range of 0.14—0.7 nmol/l, the average in our region was 0.42 nmol/l, which we took as the norm. In men, it ranged from 0.03 to 0.19 nmol/l, which averaged 0.11 nmol/l. This level was considered to be the normal level of estradiol in men. Results and discussion. Established, that in men with firstly diagnosed tuberculosis and with the sensitivity of Mycobacterium tuberculosis to antibacterial drugs, the level of estradiol in the blood increased to 0.27 nmol/l, which exceeded the norm by 2.5 times. At MDR-TB cases, it increased to 0.44 nmol/l, exceeding the norm by 3.5 times and indicates the involvement of additional reserves of the body in the fight against TB infection. In women with firstly diagnosed tuberculosis with susceptibility of Mycobacterium tuberculosis, the level of estradiol decreased to 0.2 nmol/l, i.e. 2.1 times, which may indicate a deterioration of the body's defenses, and at MDR-TB there was an increase in the level of estradiol in 1.4 times — to 0.6 nmol/l. It may indicate tension of the body's protective forces in order to overcome the tuberculosis infection. Conclusions. The authors express the hypothesis that the level of estradiol affects the resistance of an organism to tuberculosis infection — both sensitive to antibacterial drugs and resistant, which is confirmed by a lower incidence in women.
