Evaluation of the effects of P6 acupressure application on postoperative nausea and vomiting during laparoscopic cholecystectomy

Abstract
Background: Postoperative nausea and vomiting (PONV) are most common and distressing complication of anesthesia. Acupressure at P6 acupoint is non-pharmacological method which can reduce postoperative nausea and vomiting (PONV). The purpose of this study was to investigate the effectiveness of P6 acupressure in patients with known risk factors for PONV. Methods: Hundred patients of age group 25-50 years, ASA grade I and II undergoing laparoscopic cholecystectomy under general anaesthesia, were enrolled in this prospective, double-blind, randomized controlled trial. Patients were sub divided into two groups: group I and group II, 50 patients in each group II. Group I received Psi band and group II received sham band (dummy acupoint stimulation), on bilateral wrists. Ondansetron 4 mg given preoperatively and before completion of surgery, as an antiemetic prophylaxis. Fentanyl 1mc/kg administered at the end of surgery for postoperative pain management. Incidence and severity of postoperative nausea and vomiting, requirement of rescue treatment, additional antiemetic, and patient satisfaction were recorded for 24 h after operation. Results: Incidences of PONV and rescue treatment requirement were significantly lower in group I. Rate of PONV was 28% in group I and 52% in group II. Among them 11 patients (22%) required rescue treatment in group I and 23 patients (46%) in group II (p value < 0.05). Additional antiemetic required in 2 patients (4%) in group I and in 6 patients (12%) of group II. Mean NVS score was significantly higher (1.36 + 1.52) in group II compared with group I (0.66 + 1.17) (p value <0.05). Patient satisfaction score is better with group I, 2.38 + 0.66 vs. 1. 72 +0.67 in group II (p value < 0.05). Conclusion. Acupressure at P6 acupoint can reduce incidence and severity of PONV significantly in high risk patients undergoing laparoscopy cholecystectomy under general anaesthesia.
