EVALUATION OF THERAPY WITH MCKENZIE METHOD IN PATIENTS WITH THE ROOT-CONFLICT IN THE LUMBO-SACRAL SPINE BASED ON RESEARCHES OF ELEKTRONEUROGRAPHY AND SENSORY SUPERFICIAL PERCEPTION WITH VON FREY’S FILAMENTS

Abstract

Introduction. The consequences of disc herniation in patients with lumbosacral disc-root conflict are usually changes in the transmission of motor fibers in L5-S1 ventral roots as well as in tibial and peroneal nerves peripherally. They are frequently conduction blocks of a neuropraxia type. This pathology can be confirmed by testing of M and F waves parameters in electroneurography studies. Pressure on the dorsal roots at this level produces deficits in sensory nerve conduction, which somatotopically can specified as analgesia in the study of sensory perception evaluation with von Frey’s filaments according to dermatomal innervation. Aim. Using above methods, the pilot study was carried out to determine whether McKenzie therapy improves the motor and sensory transmission in patients with lumbar spine discopathies. Material and methods. In 5 patients (aged from 26 to 69 years) with a root- conflict at the level of L5-S1, the pilot clinical neurophysiology tests were performed twice, before and after 2 weeks of treat ment according to McKenzie method. Results. Before treatment, in 3 out of 5 patients, the pathological changes in the ventral roots motor fibers with consequences in peripheral transmission were found. These pathologies did not significantly changed after treatment when testing M-wave parameters. These changes were found in recorded F waves. Bilateral analgesia in the areas of the foot dorsum and the calf was diagnosed before treatment in 3 out of 5 patients, and it was detected again in 2 patients after the therapy. Conclusions. Studies indicate the direct positive effect of applied therapy after 2 weeks with regard to ventral roots decompression, without specifying the improvement in the motor and afferent transmission peripherally.
