Factors associated with default from treatment among patients of pulmonary tuberculosis

Abstract
Poor treatment adherence is a serious problem in tuberculosis control that leads to treatment failure, relapse, death, persistent infection and risk of drug resistance. The objective of the study was to identify factors associated with default from treatment among patients of pulmonary tuberculosis. A retrospective case control study design was used. Cases were those who defaulted from treatment and Controls those other than defaulters, registered for treatment under Revised National Tuberculosis Control Programme between January 2013 and December 2013. A total of 130 cases and 126 controls were enrolled for the study. Data was analyzed using SPSS software. Univariate and multivariate logistic regression analysis to determine association of factors with default from treatment were applied. Univariate analysis of factors indicated that male gender (OR 2.39; 95% CI 1.27- 4.40), Sputum smear positivity for Acid Fast Bacilli (OR 5.14; 95% CI 2.7-9.44), Alcoholism (OR 2.02; 95% CI 1.23-3.34), unemployed (OR 1.81; 95% CI 1.10-2.98), inadequate knowledge about Tuberculosis (OR 2.18; 95% CI 1.19-4.02) and regression of symptoms (OR 2.18; 95% CI 1.14-4.16) were significantly associated with default from treatment. Sputum smear positivity (a OR 5.26; 95% CI 1.36 – 20), inadequate knowledge about Tuberculosis (a OR 2.38; 95% CI 1.23 – 4.54) and regression of symptoms (a OR 2.56; 95% CI 1.26 – 5) were independently associated with default from treatment on multivariate logistic regression analysis. Factors associated with default from treatment should be address in tuberculosis care and policy actions to tuberculosis control. Appropriate counseling about tuberculosis disease and treatment should be given to patients, in order to promote adherence to treatment.
