From headgear to implants - development of distalization methods in a treatment of class II malocclusion

Abstract

AIM: The aim of the study was to present methods ofmolar-distalization in Class II treatment.MATERIAL AND METHODS: The study based onPolish and foreign literature-review evaluating efficacy ofdifferent methods applied for distalization of molars. Theeffects of headgear, upper removable appliance and fixeddevices like Keles Slider, Jones Jig, Distal Jet and Pendulumwere presented. Possibilities of absolute anchorage -palatal implants and micro-implants - application for molardistalization were also discussed.RESULTS: In studies on treatment effects of headgearand upper removable appliance authors assumed that thecorrection of Class II relation was brought about by increasedsagittal growth of mandible. The intra-arch devicesthat have almost eliminated patients’ compliance wereestimated by investigators as effectively distalizing maxillarymolars appliances but they all produced a certainamount of anterior anchorage loss and distal molar tipping. Palatal implants supporting modified Distal Jet orPendulum appliances enabled molar Class I establishmentwith no anchorage loss in anterior segment of an archwire.However, titanium microimplants provided efficient anchoragefor backward movement of not molars only but ofwhole maxillary dentition as well.CONCLUSIONS: Distal bodily movement of molarsmay be difficult to achieve with headgear, since this therapyis time consuming and requires patients’ compliance.On the other hand, whenever intraoral devices are applieda certain side effect is involved - anchorage loss. Thereforemicroimplants seem to be promising solution reducingundesired effects associated with Class II malocclusiontreatment. (Orthod. Forum 2005; 2:10-15)
