Health Care Workers: A Potential Source of Methicillin and Multi Drug Resistant Coagulase Negative Staphylococci

Abstract

Context: Coagulase negative staphylococci (CoNS) have recently emerged as an important cause of nosocomial infections and reservoirs of drug resistance genes for Staphylococcus aureus. Health care workers (HCWs) carrying CoNS can be a potential source of infection to the patients they take care of. Aim: The present study was done to know the prevalence of nasal carriage, methicillin resistance and antibiotic susceptibility pattern of CoNS among health care workers. Materials and methods: Nasal swabs were collected from 102 health care workers (HCWs) of a tertiary health care hospital and processed immediately on sheep blood agar. CoNS isolated were tested for methicillin resistance and resistance to other drugs by Kirby-Bauer disc diffusion method according to CLSI 2011 guidelines. Results: Overall nasal carriage of CoNS and methicillin resistant CoNS (MRCoNS) was seen in 58(56.86%) and 42(41.17%) of the HCWs respectively. Difference in carriage rate of CoNS and MRCoNS between different HCWs was statistically not significant but was significantly higher in females. Drug resistance was very high among MRCoNS isolates when compared to methicillin sensitive CoNS(MSCoNS) isolates. All isolates were susceptible to Linezolid. Conclusions: There is a very high prevalence of nasal carriage of CoNS and MRCoNS in HCWs. Drug resistance is very high among MRCoNS isolates. Linezolid has good anti CoNS activity. Careful infection control practice, judicious use of antibiotics, frequent hand washing, periodic screening with decolonization of carriers should be considered in health care centers.
