Hematological parameters in HIV subjects on Zidovudine Therapy

Abstract
Background: Hematological parameters are important monitoring tools for assessing treatment and prognosis in HIV. Choice of antiretroviral drugs could positively or negatively affect these parameters. 1 Thus a specific diagnosis and a determination of hematological parameters and CD4 count are required for initiating and monitoring early treatment to avert disease progression. Materials and Methods:A prospective study of Hematological parameters in 25 HIV positive patients were analyzed before and after initial antiretroviral therapy by Zidovudine Regime at Anti Retroviral Therapy (ART )Center, for a period of eight months from 1 Dec 2013 to 31 Jul 2014 . This study aims to compare hematological parameters and CD4 Counts specifically in Zidovudine Regime. Majority of these patients were in WHO Clinical Stage III (56%) and all of them had a CD4 count of < 200 per/µl. Anemia and cytopenias namely neutropenia, lymphopenia and thrombocytopenia have been compared before and after the initial treatment. Elevated erythrocyte sedimentation rate by Westergren’s method have been recorded in the defined time interval for all the subjects. Results:The commonest hematological abnormalities in Zidovudine treated HIV were anemia, leucopenia and thrombocytopenia. Prevalence of anemia was high in Zidovudine treated patients in comparison to other cytopenias. Thrombocytopenia is more prevalent than leucopenia in the cohort studied. Low CD4 count coexists with anemia and cytopenias. Conclusion:HIV patients on ART develop more severe clinicopathological manifestations of hematological abnormalities. Consequently, the development and assessment of indigenous antiretroviral drugs with minimal abnormalities becomes a necessity. In addition to clinical follow up, a regular monitoring for demonstrating treatment failure is essential as disease progression itself is associated with the development of anemia.
