Immunohistochemical and morphometric study of duodenal biopsies from school-aged children with a previous history of cow's milk allergy

Abstract
Introduction: Adverse reactions to food that result in gastrointestinal symptoms are common in infancy. Recently formal evidence have been found that cow's milk protein sensitive enteropathy may persist in older children. Aim of study: The aim of the study was an immunohistochemical and morphometric study of duodenal biopsies from school-aged children with a previous history of cow's milk allergy. Material and methods: The study covered a consecutive series of the children referred due to recurrent abdominal pain: 30 children with cow's milk allergy in infancy (group A), 56 children with abdominal pain without food allergy (group B)and 20 children diagnosed for short stature and anemia (group C). Ali children were currently on normal diet. During endoscopy biopsy specimens were taken from the descending duodenum in all subjects. Sections were routinely stained with HE for mucosal histology and for CD3+, CD8+, CD20+, tryptase and Ki67. All subjects were also investigated for serum concentration of food-specific IgE. The study protocol was approved by the local research ethic committee. Results: In children with cow's milk allergy of infancy an increase of CD3+ mononuclear cells in duodenal mucosa was observed. No significant differences in basie histology includingvillous or erypt height and in eosinophilic leucocytes, CD8+, mast cells counts in duodenum were observed as compared to the controls. Conclusions: The inereased number of CD3+ mononuclear cells in duodenum of the children with a previous history of cow's milk allergy may suggest an enhanced local immune response in the Cl mucosa.
