Implants treatment after impacted canine surgical removal and allogenic bone transplantation supported with A-PRF

Abstract
Introduction. Impacted permanent teeth within the anterior segment of the maxilla constitute both, an aesthetic and a medical problem. The treatment of choice comprises orthodonticsurgical procedures consisting in guiding an unerupted tooth into occlusion. However, in some cases implant-prosthetic treatment is necessary. Study objective. This study is aimed at efficacy evaluation of augmentation with allogenic bone as preparation for implant-prosthetic treatment in places of removed impacted canines. Material and methods. The study involved 9 patients with an impacted canine qualified for extraction for different reasons. Directly after extraction the alveolus was augmented with allogenic bone. Additionally, graft remodelling was facilitated with the use of PRF membranes. Results. In all the cases bone loss at the ridge after the impacted tooth was extracted prevented primary stability of the implant within its coronal section. Therefore, the height and/or width of the alveolar ridge was expanded in two stages: extraction of the impacted tooth and simultaneous bone augmentation and then, after 6 months, intraosseous implantation. A follow up radiological examination performed after a 6-month graft healing period showed normal structure of hard tissues within the surgical site and a volume of the ridge that would allow implant placement. The outcome of the histopathologic examination confirmed graft remodelling into a patient’s own bone. In each case the treatment was completed with implant-prosthetic restoration. Conclusions. Implant – prosthetic treatment with prior bone augmentation is an effective alternative if an impacted tooth cannot be guided into occlusion using orthodontic-surgical procedures. In view of the material gathered by the authors the most important factor is adequate planning of the surgical tactic and using biomaterial of optimal consistency.
