Interdisciplinary treatment of patient with face asymmetry, open bite and agenesia of maxillary lateral incisors – a case report

Abstract

The aim of the study was to present interdisciplinarytreatment of a patient with face asymmetry, completeopen bite and two maxillary lateral incisors congenitallymissing. A case of a 19.6-year-old male, who sought orthodontictreatment because of difficulty tearing foodand appearance discomfort, was described. Extra-oraland intra-oral examinations were performed and diagnosticmodels and radiograms studied. Clinical examinationrevealed elongated lower face and marked chin displacementto the left. Intra-oral examination revealed Models demonstrated constricted maxilla and slightcrowding of lower incisors. Cephalometric analysis revealedmaxillary protrusion (SNA angle) with retractedmentum (SNPg), steep mandibular plane positioning(SN/ML), increased anterior face height (NMe) in relationto posterior height (SGo). Angle between incisor`saxis and mandibular base (-1/ML) was decreased. Orthodontictreatment was performed first with quadhelixand then upper and lower fixed orthodontic appliances.Mandibular third molars` extractions, bilateral osteotomyof the mandibular branches and shaft as well as genioplastywere performed by dental surgeon. Fixed appliancewas removed after 22 months. Esthetic dentistryspecialist performed recontouring of maxillary caninesto make them resemble lateral incisors. Combinedorthodontic, surgical and esthetic treatment resultedin obtaining satisfactory functional and esthetic results.(Orthod. Forum 2008; 4: 13-20)
