Isolated Torsion of the Fallopian Tube - Suspicion is the Key to Success

Abstract

Adnexal torsion is one of the few gynaecological emergencies presenting with non specific clinical symptoms and hence, are rarely suspected and diagnosed in time. An acute episode of lower abdominal pain associated with nausea and vomiting can clinch the diagnosis of adnexal torsion but isolated tubal torsion if present, still comes as a surprise to the surgeon. We report a case of a 20 year old girl who presented with acute abdominal pain and a simple anechoic cyst in the right adnexa with intact vascular supply. On laparoscopy, it was a case of isolated tubal torsion with a large hydrosalpinx, laparoscopic salpingectomy was done as the tube was grossly damaged. This case highlights that tubal torsion should be included in the differential diagnosis of acute abdomen in females of all age groups, in order to timely intervene and prevent permanent damage to the patient's fertility.
