La thyroglobuline plasmatique chez la femme enceinte à Lubumbashi Analyse de son profil et de ses relations avec la TSH, l’Iode urinaire et la ferritine

Abstract

Background: indicators for iodine nutrition stutus are goiter rate, urinary iodine excretion and rate of household using iodide salt. Thyroglobulin has been used as a biomarker for iodine status mainly in children. No data in our area exist for pregnant women. This study aimed to determine thyroglobin profil during pregnancy and its variations according other parameters such TSH, urine iodine and Serum ferritin Method: cross sectional study including 225 pregnant women at first, second and third trimester. We collected urine samples in sterile box for iodine assay using ammonium persulfate method. TSH, Ferritin and thyroglobulin was measured in serum by immunoanalysis assay. Statics was done using Epi-info 7 and Execl, with the significance threshold of 0.05; Results: the trend of thethyroglobulin was to increase with gestational evolution (r=0.18, p=0.04). There was a significant difference betweenthyroglobulin median during trimester: 9.2 ng/ml (3.5-16.5 ng/ml) at first trimester, 14.2 ng/ml (7.2 – 23.7 ng/ml) at the second trimester and 16 ng/ml (7.3- 25.4 ng/ml) at the third trimester. 52% of pregnant women had low iodine urine excretion (<150 µg/l) with 23% of them having less than 50 µg/l). In the presence of Iodine deficiency, the thyroglobulin median was high. The trend of thyroglobulin was to decrease when iodine status was normal and to increase when TSH level was upward; it increased too when ferritin level was low. Conclusion: thyroglobulin seems to be a good iodine status biomaker good biomarker among pregnant women and it’s reflect good thyroid gland adaptation to incoming iodine nutrition
