Large Demoid Cyst Complicating Pregnancy

Abstract

The coexistence of an adnexal mass with pregnancy presents problem to both clinician and patient. The most serious being that of malignancy. Benign tumors are more common. Incidence depends upon whether simple cysts on USG and clinical examination are considered. Those that ultimately require laprotomy are in 1 in 1000 to 1 in 1500 live births. The most common benign neoplasm of the ovary in pregnancy is the Benign Cystic Teratoma, one third of all ovarian tumors. Name: K. Swapna Sri/ W/O K. Naveen, 24 years R/O of Chintal, Qutbullapur, Medchel District, TS DOA: 28/5/2018 DOS: 9/6/2018 IP NO: 18052800029 Primigravida ML: 1 year First visit to our hospital was on 28/4/2018. All the routine Antenatal investigations showed normal values. Informed consent: Informed consent was obtained from the patient for presentation of case report. Obstetric USG Scan showed single live intrauterus fetus of corresponding to 33 week 4 days +-2 weeks with good cardiac activity with cephalic presentation, adequate liquor, placenta in fundus posterior wall Grade III maturity. Cyst measuring 92x68x92 mm noted in right adnexa adjacent to uterus with internal foci. No septation right ovarian cyst. Patient had regular antenatal check up weekly. Elective LSCS was done on 29/5/2018. She delivered a male baby of weight 2.7 kgs. Vigorous placenta with membrane delivered out. Uterus was closed in two layers. UV fold was closed. Adenexa were examined. Both tubes were normal. Right ovary showed large cyst with ovarian tissue. Left ovary was normal. Sebaceous material was found after opening of right ovarian cyst. It was preceded with oophorectomy. Hemostasis was procured. Abdomen was closed inlayers. One course of antibiotics was given. Post operative period was smooth. Suture removal was done & discharged on 10th post operative day. Histopathology showed Desmoids cyst.
