Les Plaintes Proctologiques Chez Un Echantillon De La Population Generale Marocaine

Abstract

Aim: To investigate about practices related to proctological complaints in Morocco. And to specify there prevalence. Materials and methods: It is a study of a sample of 206 adults from the Moroccan population at random people in public places, responding to a pre-questionnaire. Results: The mean age was 42.39 years, sex ratio of 1.64 (H / F). 133 lived in urban areas (64.6%) and 73 lived in rural areas (35.4%). Educational level was primary in 28.2%, secondary 36.9% and 26.7% in university. 8.3% of those recruited were analphabet. Sixty seven (32.5%) have never had a complaint against 139 other (prevalence 67.5%). These symptoms were dominated by constipation (27.72%), proctalgia (24, 91%), and rectal bleeding (23.16%). Symptomatic persons (n = 139) were younger than non-symptomatic (n = 67) (mean age, respectively: 38.3 years vs. 44.7 years, p = 0.0039). In addition, living in an urban zone is associated with the presence of symptoms (p = 0.00054). Of the 139 symptomatic persons, 80 never consulted a physician (57.55%) because they do not see the need to consult in 58.75%, from modesty in 13.75% or because of a combination of both reasons in 27.50%. Self-medication was reported following a recommendation from a friend or family member (77.50%), a pharmacy staff (15%) or from websites (7.50%). Treatments received were in the form of tablets (16.25%), suppositories (20%), ointment (11.25%) or diet alone in 12.5% of cases. Thirty-eight people (40%) admitted using traditional recipes as a remedy. Fifty nine persons consulted a physician (42.45%), it is a general practitioner (52.54%), a gastroenterologist (37.29%) or a gynecologist (10.17%). Twenty persons (33.9%) did not have a proctology exam, or by decision of their doctors (55%) or because they refused in 45% of cases. The refusal was in two thirds of cases because of the difference in sex doctor, or by modesty in the other third of cases.The Examination in thirty-nine persons (66.1%) was a simple inspection in 28.20%, a digital rectal examination in 30.77%, and full proctological exam with anorectal endoscopy in 41.03%. All gastroenterologists reviewed their patients. While about half of general practitioners and gynecologists did not. The evolution of symptoms in people who looked and in others that have not was: the disappearance (45.76% vs. 6.25% respectively), improved (45.76% vs 46 25% respectively) and persistence (47.50% vs. 8.48% respectively). Conclusion: The frequency of proctological complaints in our population was elevated, the consultation is not the rule in this sense, replaced by self-medication.
