Management of spontaneous hemothorax and hemo-pneumo thorax

Abstract
Spontaneous hemothorax corresponds to an accumulation of blood in the pleural cavity outside of any traumatic context. It is a rare clinical entity whose etiologies are generally reported in the form of clinical cases or series of sporadic cases especially in Anglo-Saxon literature. The clinical presentation varies from a completely asymptomatic patient to a haemorrhagic shock. Indeed, the consequences of the respiratory and hemodynamic dysfunction that accompany these hemothorax are likely to lead to the prognosis and/or aggravate the prognosis of the underlying pathology. This justifies a rapid response with the adoption of a comprehensive therapeutic strategy combining an assessment of severity, appropriate guidance, symptomatic management (oxygen therapy, transfusion) and etiologic management. The majority of these etiologies are poorly known not only to pulmonologists but also to thoracic surgeons who must nevertheless be the main actors in the management of these haemothoraxes whose diagnosis and management will be described along this article.
