We report a case of a 15-year-old male who presented with a history of non-specific gastro-intestinal symptoms such as intermittent and recurrent bilious vomiting, chronic intermittent abdominal pain since last two years. The patient was thoroughly investigated. An upper GI barium study and contrast enhanced CT scan of the abdomen suggested midgut malrotation along the axis of the superior mesenteric artery, On basis of imaging studies, pre-operative diagnosis of midgut malrotation was made. The patient underwent a laparotomy. Ladd’s procedure was performed. The postoperative period was uneventful. The patient was successfully discharge on day 10 and subjected to follow up.
