Modern methods of therapy of hard-to-heal post-operative wounds in obstetrics and gynecology – analysis of applicability and effectiveness of use

Abstract

Impaired healing of post-operative wound constitutes a significant medical problem worldwide. Infected, chronic and hard-to-heal wounds leading to dehiscence of the surgical site layers expose patient to the development of numerous complications, prolong hospital stay and result in additional costs of treatment. In relation to obstetrics and gynecology and the two most commonly performed surgical procedures – abdominal hysterectomy and cesarean section rate of infected post-operative wounds is reported to be as high as 1.8-11.3% and in approximately 0.3-1.2% of all cases subsequent wound dehiscence occur. As a result new methods of post-operative wound treatment in obstetrics and gynecology need to be explored, which will replace traditional dry dressings. In the last twenty years in the literature there have been numerous reports of novel strategies in the treatment of chronic and hard-to-heal wounds which additional properties allow them to stimulate natural processes of tissue reparation, provide an appropriate environment for the healing process and protect wound bed against the development of infection alongside covering of the wound. Preparations of platelet-rich plasma and cellular growth factors, negative-pressure wound therapy and antibacterial silver-containing dressings are examples of these alternative methods. Based on a review of available literature presented paper discusses the applicability and effectiveness of aforementioned methods in the treatment of hard-to-heal post-operative wounds in obstetrics and gynecology.
