PATOPSYCHOLOGICAL FEATURES OF PATIENTS WITH RHEUMATOID ARTHRITIS AND NONPSYCHOTIC MENTAL DISORDERS

Abstract
Objective. Rheumatoid Arthritis (RA) is a chronic progressive autoimmune disease, with a worldwide adult prevalence of 0.2-1.2 %. We aimed to study the individual psychological features of patients with rheumatoid arthritis and non-psychotic mental disorders Material and Methods. One hundred and ninety patients with a diagnosis of Rheumatoid arthritis, according to the criteria of International Classification Disease 10 (ICD), who attended clinics for follow-up visits between June 2011 and November 2016, were included in this study. The study was approved by the ethics committee of the hospital, and signed informed consents were obtained from patients. The patients with a diagnosis of rheumatoid arthritis and nonpsychotic mental disorders was evaluated using the abbreviated method of studying the personality of a mini-mult (adaptation of F.B. Berezin and M.P. Miroshnikov) and questionnaire of diagnosing character accentuations (K. Leonhard, G. Shmyshek). This group of methodic permitted to mark out individual-psychological features of personalities. Results. Of the 120 patients, 91 were female (75.3 %) and 19 were male (15.7 %); the mean age was 47.51±8.71 years. According to patients’ perception, the most frequent reasons for joint symptoms were nonpsychotic mental disorders (NMD). Patients group A with duration of RA 1-5 years often had the emotional type of accentuation: 13.2 ± 0.75 and disthymic type – 14,1 ± 1,01, but patients group B with duration of RA of RA 5-10 years often had emotional type of accentuation: 14.0 ± 0.67 and cyclothimic type – 13,2 ± 0,72. Conclusion. Nonpsychotic mental disorders included anxiety-depressive disorders were found with 55 patients (45,8 %), disorders of adaptation – 25 patients (20,8 %), somatoform disorder – 21 patients (17.5 %), anxiety-phobic disorders – 14 patients (11.7 %), obsessive-compulsive disorder – 5 patients. Patients group A with duration of RA 1-5 years often had the emotional type of accentuation: 13.2 ± 0.75 and disthymic type – 14,1 ± 1,01, but patients group B with duration of RA 5-10 years often had emotional type of accentuation: 14.0 ± 0.67, anxious type of accentuation (13,7 ± 0,86) and cyclothimic type of accentuation – 13,2 ± 0,72.
