PREVALENCE OF DYSLIPIDEMIA IN TYPE 2 DIABETIC PATIENTS: A STUDY IN CCM MEDICAL COLLEGE AND HOSPITAL, CG

Abstract

Prevalence of obesity, hypertension, diabetes, and dyslipidemia is closely associated with the cardiovascular epidemic worldwide. People living with diabetes can be classified as high risk patients for cardiovascular events. Therefore, irrespective of presence or absence of other risk factors on history or examination. Dyslipidemia is a classical constituent of metabolic syndrome and the underlying mechanism for development of hyper-lipidaemia in the metabolic syndrome has been clearly established. Dyslipidemia in diabetes generally manifests as elevated low-density lipoprotein cholesterol (LDL-C), decreased high-density lipoprotein cholesterol (HDL-C) levels, or elevated triglyceride (TG) levels. Most of the guidelines recommend management of lipids in this population. Materials and Methods: Demographic data of participants like age, sex, monthly income, life style, family history of diabetes, and prior diseases/disorders history were recorded. Fasting blood samples were collected in EDTA vial and plasma was separated by centrifuging the blood samples. Total cholesterol, triglyceride, and HDL-C levels were estimated, Low- and very-low-density lipoproteins were calculated, and triglyceride and HDL-C level were used as parameters for diagnosing of dyslipidemia. Results: A total of 88 consecutive patients with type 2 diabetes were enrolled in the study. Of the 88 patients 54 were male and 34 were female patients. Mean age of male patients in the study sample was 51±11.2 and female was 57±13.4 (P = 0.0015, 95% CI =2.3256 to 9.6744), mean fasting blood sugar level in male and female patients was 145 ± 59.4 and 151±55.2 (P = 0.4885, CI -11.0608 to 23.0608).Mean Body mass index (BMI) in selected male patients was 25.3 ± 3.6 and in females was 26.7± 4.2 (P = 0.0187 CI 0.2361 to 2.5639). Prevalence of dyslipidemia in male patient was 88.9% while in female patient it was 79.4 %. Of the total 51 urban patients dyslipidemia was seen in 90.2 % while in rural patients it was 78.4 %.of the total 56 obese and 32 non obese patients dyslipidemia was 89.3% and 78.1 %. There were 12 patients having duration of diabetes less than 2 years and out of these 58.3% were having dyslipidemia. Total mean cholesterol level was 179 ± 48.9 and in 49 (55.7%) patients total cholesterol levels were increased. Mean Triglyceride was 45.6±18.2 with high levels were seen in 74 (84.1%).Conclusions: Majority of type 2 diabetic patients are dyslipidemia at baseline. The most common pattern of dyslipidemia is high Triglyceride, LDL and low HDL. So the aggressive dyslipidemia management is the needed for the patients with diabetes.
