Prevalence of Primary Open Angle Glaucoma in diabetic patients visiting a tertiary eye care hospital.

Abstract
Aim : Background: The relationship between type 2 diabetes mellitus (DM) and primary open angle glaucoma (POAG) is still unclear. POAG is often an asymptomatic disease till late. Understanding the risk factors associated with the development or progression of POAG will help clinicians identify patients who will benefit from screening and careful monitoring of the disease. This study was undertaken to find out the prevalence of POAG and to evaluate the risk factors in diabetic patients visiting a tertiary eye care hospital. Materials and Methods: A total of 1636 patients (3272 eyes) having type 2 DM were included in the study. All of them underwent a detailed ophthalmic and medical history taking, random blood sugar (RBS) estimation and blood pressure recording. Preliminary eye examination including recording of visual acuity, silt lamp examination, measurement of intra ocular pressure (IOP) and fundus evaluation were carried out. Patients having an IOP of > 21mmHg were considered for further evaluation. Results: The incidence of glaucoma in type 2 DM patients in our study was 6.81%. Patients having RBS > 140mg% shared a greater risk of developing glaucoma ( adjusted OR 1.373; 95% CI 0.749 - 2.516). Hypertension was also a significant risk factor for glaucoma ( adjusted OR 1.767; 95% CI 1.036 - 3.015). Conclusion: All patients with type2 DM should be evaluated for glaucoma to prevent irreversible blindness. Attention should also be given to other major systemic disorders.
