PROFILE OF MULTIPAROUS WOMEN UNDERGOING PRIMARY CAESAREAN SECTION

Abstract

BACKGROUND Caesarean section is one of the most common surgeries performed in modern obstetrics. It is defined as the birth of a foetus via laparotomy and through hysterotomy. Caesarean section done for the first time in a woman is called Primary caesarean section, while caesarean on a uterus having one or more prior hysterotomy incisions is called secondary. Primary caesarean section in multiparous woman refers to caesarean section done for the first time in a lady who had one or more previous vaginal deliveries. The common indications for this procedure are foetal distress, malpresentation and antepartum haemorrhage. We wanted to find the prevalence of primary caesarean section in multiparous women and analyse the indications for primary caesarean section in them. METHODS Hospital records of multiparas who underwent primary caesarean section were collected from medical records library. From each case record, demographic data, antenatal complications, type of caesarean section, indication, intra operative and postoperative complications and maternal and neonatal morbidity were analysed to calculate the prevalence and to identify the major indications. The data was then analysed. RESULTS During the study period, the prevalence of primary caesarean section in multipara included in the study was 13.11%. Most of them were para 1, belonging to the age group of 26 - 30 years. Majority of them were booked cases. The most common indication for CS was foetal distress (21.64%). 83.93% CS were done as emergency procedures. Atonic postpartum haemorrhage was seen in 18 cases and post-operative fever was seen in 3.27%. CONCLUSIONS Multiparous women are also at increased risk of primary caesarean section. Early identification and treatment of risk factors leading to CS like macrosomia, cephalopelvic disproportion, gestational diabetes and hypertension can reduce maternal and foetal morbidity. Efficient antenatal care and careful intrapartum monitoring help in reducing complications in mother and child.
