Quality of root canal fillings in radiographic examinations

Abstract
Success or failure of endodontic treatment mainly depends on the quality of root canal fillings. In clinical conditions, radiographic or radiovisiographic (RVG) examination as well as computed tomography (CT) are the only methods of imaging filled root canals. The aim of the study was to evaluate the quality of root canal fillings basing on the analysis of RVG and cone-beam computed tomography images. Material and methods: Sixty-four roots of extracted human teeth were used in the study. The teeth were randomly divided into 4 groups (n=16). The canals were instrumented with ProTaper® rotary files. During the last canal rinsing, NaOCl and EDTA solutions were activated with a Satelec® ultrasound scaler. The teeth were filled with the following methods: Group 1 with the greater taper single cone technique; Group 2 with the cold lateral condensation technique; Group 3 with the ProTaper® Thermafil obturator technique; Group 4 with the continuous wave warm vertical compaction technique using an Elements Obturation Unit®. Epoxy resin-based AH Plus® sealer was used in all groups studied. RVG and CBCT images were taken to radiographically evaluate root canal fillings. Results: 1. Thermal methods of root canal obturation more frequently resulted in an unintentional displacement of the filling material beyond the root apex as compared to the technique of lateral condensation of cold gutta-percha. The Thermafil obturator technique more frequently caused pushing the filling material beyond the root apex in comparison to the cold lateral condensation and the combined method of warm vertical condensation with gutta-percha injection. 2. No statistically significant differences were observed in the number of voids and in filling root canals after using cold gutta- percha and thermal methods. Conclusions: In in vitro studies of one-rooted teeth, the image of root canal fillings using RVG and CBCT techniques is comparable.
