RESULTS OF TREATMENT THE SHOULDER INJURIES USING ARTHROPLASTY

Abstract

Introduction. Replacing the elements of shoulder on the prosthetic components is an effective way of treatment in cases of its various injuries. The success of treatment depends on many factors such as the patient’s preparation, the surgical technique used, the biological status and the potential of repairing tissues, the proper post-operative care and rehabilitation process. Aim. The aim of the study was to summarize the results of the shoulder lesions treatment using the arthroplasty. Material and Methods. The study involved 25 patients: 15 women and 10 men. Indications for surgical procedures included: osteonecrosis (AVN; 4 patients), primary or secondary degenerative changes (OA, 15 patients), fractures within the glenohumeral joint (FR, 8 patients). The study consisted of two stages: assessment of range of motion, pain intensity and function of the shoulder before arthroplasty and evaluation of these parameters in the control study. For each patient, there were measured the range of flexion, external rotation and abduction of the shoulder joint. There were evaluated the level of pain using the VAS (Visual Analogue Scale) and the range of upper limb function using the UCLA scoring system. Results. In all groups of patients undergoing the arthroplasty in the shoulder there were obtained improvements regarding the range of motion, function and the pain intensity reduction. Groups of patients with osteonecrosis had the best results for the parameters evaluated preoperatively and obtained the greatest improvement in the control study. The worst results for the range of motion and upper limb function were observed in patients after shoulder fractures. Conclusions. The treatment of shoulder joint with arthroplasty is an effective way to treat pain and limitation of function in cases when the conservative treatment is ineffective.
