Retrospective assessment of qualification criteriaand efficasy of azathioprine therapyfor ulcerative colitis in children - experience of single center

Abstract
Aim of study: The aim of this study was the assessment of qualifications and efficacy of azathioprine (AZA) treatment in children with ulcerative colitis (UC). Material and methods: 25 patients treated for UC with azathioprine were enrolled to the study (14 females and 11 males, aged from 1 to 17 years). Patients' data were retrospectively analyzed. In all cases azathioprine was administrated because of failed steroid and 5 aminosalicylic compounds therapy. 10 patients (40%) were previously treated with prednisone and sulfosalazine, 4 patients (16%) received pre-dnisone and mesalazine, 5 (20%) sulfasalazine alone. Steroids and aminosalicylates was resulted in neither clinical (n=25, 100%), nor en-doscopic(n=9, 35%) and histopathological (n=5, 20%) improvement. Azathioprine treatment was instituted in 8 children (32%) duringfirst exacerbation of UC. Remaining patients got this therapy for second or another exacerbation. Results: AZA therapy resulted in improvement of clinical, endoscopic and histopathological status in 20 patients (80%). Exacerbation of UC was observed in 7 cases (28%) before 6 months of AZA therapy. The azathioprine was weaned off in two patients (5%) because of thrombocythopenia and in one (2.5%) because of acute pancreatitis. 22 patients (88%) didn't present sideeffects of AZA. Cyclosporine therapy was started because of severe exacerbation of UC in 10 patients (40%), in 4 children (16%) in this group colectomy was performed. 7 patients (28%) have been still receiving AZA. Conclusions: In analyzed group AZA therapy was started because of steroid resistance and steroid dependence. AZA seems to be safe and well tolerated treatment of UC in children.
