REVIEW OF MODERN APPROACHES IN THE DIAGNOSIS AND TREATMENT OF NECROTIZING FACIITIS

Abstract
Necrotizing fasciitis is a severe rapidly extending surgical infection of superfi cial fascia and subcutaneous fat which leads to the development of secondary skin necrosis, severe sepsis and multiple organ failure. If local symptoms of the patient don’t coincide with his general state, all the possible assistants should be used – from non-invasive imaging diagnostic to diagnostic wound exploration for confi rmation or denial of morphological substrate necrotizing fasciitis. It is accompanied with high rates of mortality and limbs loss It is accompanied with high rates of mortality and limb loss. Modern approaches to the early diagnosis and effective surgical debridement of necrotizing fasciitis are reviewed in the article. The principle of surgical debridement is proposed, which depends on the advance of pathomorphological changes in the soft tissues. The use of this approach allows to decrease the rate of diagnostic mistakes and improve treatment result of this pathology. The use of modern equipments and materials for wound treatment by subatmospheric preasure has considerably improved the quality of treatment for patients with necrotizing fasciitis. It also accelerate wound healing and reduces needs for d-bridement.
