Role of appendicectomy in relieving the chronic or relapsing right iliac fossa pain at tertiary health care centre

Abstract

Background: Right lower quadrant pain is responsible for a large percentage of emergency cases and is one of the most challenging clinical presentations, with a vast list of differential diagnoses. It needs multi-disciplinary approach to diagnosis and proceed. Aim: To determine the role of appendicectomy in relieving the chronic or relapsing right iliac fossa pain. Material and Methods: This prospective observational study was conducted on 82 patients admitted with chief complain of recurrent or chronic right iliac fossa pain. All the included patients underwent appendicectomy and sample sent for histopathological examination. Postoperatively during the follow up at 6th and 12th week, they were compared with histopathology of appendix and relief of pain. Results: Out of the 82 chronic RIF cases, 30 (36.6%) had pathology of normal appendix, and 52 (63.4%) cases had pathology of appendicitis. A total of 79(96.35%) patients had complete pain relief. Mean pain score in visit 1 was 2.89 (SD±0.316) and in visit 2 was 2.98 (SD±0.156). (T value = 2.751; P value = 0.007) and it was ignificant. Conclusion: Elective appendicectomy, even a normal appendix, has pain relief in majority of patients with RIF pain. There is also correlation between histopathology of appendix and relief of pain. Appendicectomy can be recommended for chronic or relapsing right iliac fossa pain after a thorough assessment.
