Significance of serum amylase levels in diagnosis of acute severe pancreatitis

Abstract
Background: An elevation in the serum amylase is a key element in the diagnosis of acute pancreatitis, but needs to be interpreted with caution. The sensitivity of the test is also affected by the timing of the testing and the underlying cause of the pancreatitis. Aim: To evaluate the significance of serum amylase levels in diagnosis of acute severe pancreatitis. Material and Methods: A total of 50 patients with clinical evidence of AP were admitted during the study period and were analyzed by serum amylase levels. Results: Among the 50 patients, 31 patients had raised Serum Amylase level > 4 times the normal. Among these, 14 patients had Haematocrit 42 and 17 had < 42 (p value- 0.033), 4 patients had Glasgow score 3 and 27 had score < 3 (p value- 0.802), 21 patients had CT score 4 and 10 had score < 4 (p value- 0.033). Conclusion: Amylase level had high sensitivity in predicting acute severe pancreatitis. Raised serum Amylase (>4 times the normal) had significant correlation with Haematocrit (>42%) and CT index. Serum amylase still has a valuable role in diagnosis and evaluation of Acute Pancreatitis.
