Soluble urokinase plasminogen activator receptor as a potential biomarker in pneumonia

Abstract
Pneumonia is a frequent childhood disease and requires hospital treatment in 50-60% of patients under five years of age. However, despite the frequent occurrence of the problem, we are still lacking unified scales to assess the severity of the pneumonia and the prognostic factors. Thus, there are studies focusing on novel biomarkers that might be useful in stratifying the risk in patients with pneumonia and/or indicating the necessity of hospital treatment. The suPAR is a soluble form of the uPAR (CD87), which is present on various immune system cells. Research on a mice model has proved that a lack of the uPAR gene results in a more severe course of pneumonia and a worse prognosis. Elevated suPAR levels have been related with an activation of the immune system, regardless of its etiology (bacterial and viral infections, some solid tumors, autoimmune diseases). suPAR, due to the role of its precursor in inflammatory responses in patients with pneumonia, may be specifically increased in this very disease, especially in patients with a generalized infection. Previous studies on septic patients (also patients with the primary site of infection localized in the lungs) have shown significantly increased suPAR levels and their correlation with higher mortality. Although the suPAR did not show essential diagnostic values, yet, as a prognostic factor it seems promising. Moreover, its levels in severe infections are stably elevated for 7 days, thus facilitating the identification of patients that are running a higher risk of a more severe course of the disease.
