Spectrum of bone marrow aspiration and biopsy findings in cases of pancytopenia

Abstract
Background: Pancytopenia is a manifestation of many underlying disease conditions which can be life threatening if not treated promptly. This study was conducted to evaluate the etiological, clinical and hematological features with special emphasis on the spectrum of bone marrow aspiration and biopsy findings in all patients presenting with pancytopenia which is the unique feature of this study. Materials and Methods: In this study, a total of 150 cases were studied by performing complete hemograms, examining peripheral smears, bone marrow aspiration and biopsy in each case of pancytopenia. Results: Among the 150 cases studied. The age of the patients ranged from 15 to 70, with a slight male preponderance. The most common cause for pancytopenia was megaloblastic anemia followed by aplastic anemia, myelodysplastic syndrome, acute lymphoblastic leukemia, non-Hodgkin lymphoma, acute myeloblastic leukemia, multiple myeloma, chronic myelocytic leukemia and epithelial metastasis. The most common bone marrow finding was hypercellularity with erythroid hyperplasia. 8% cases were misdiagnosed as hypoplastic aspirate or dry tap on bone marrow aspiration alone which were accurately diagnosed on bone marrow biopsy. Conclusion: Pancytopenia is often a manifestation of serious underlying disease processes. Hence, Bone marrow aspiration and biopsy must be performed in all cases of pancytopenia for accurate diagnosis and to institute an early appropriate treatment.
