Stress Testing in Cervical Spondylosis: Effect of Head Up Tilt on Heart Rate Variability

Abstract
Introduction: Cervical spondylosis (CS) is a degenerative disease of cervical spine carrying significant morbidity. Autonomic nervous system (ANS) maintains homeostasis of the internal environment by a complex integration of autonomic and somatic sensory information and descending influences from higher centers. Aims and objectives: To assess the level of autonomic activity in patients with CS by studying the effect of head up tilt test on heart rate variability (HRV). Material and methods: HRV was recorded during head up tilt (HUT) and recovery period, in two groups comprising of thirty patients with CS and thirty healthy age and sex matched controls in Department of Physiology in collaboration with Department of Orthopaedics, Pt. B.D Sharma PGIMS, Rohtak. The results were analysed in time and frequency domains by using student t-test (significance level at p<0.05). Results: In group II(CS patients), LF/HF ratio was significantly increased as compared to group I(controls) during the head up tilt. During the recovery period, RMSSD(parasympathetic marker) was significantly decreased while LF (nu), sympathetic marker and LF/HF ratio (sympatho-vagal balance) remained persistently higher than the controls, implying a higher sympathetic tone in these patients. Conclusion: HRV is as a simple, non-invasive measure of the autonomic activity which analyses beat to beat variation in the heart rate and HUT test further improves the sensitivity of its interpretation. CS patients have a higher sympathetic tone than controls.
